Meal Plan Cancelation
Please complete the entire form to request special consideration for meal‑plan release.
Incomplete forms or missing documentation will not be processed.
Exemption is not guaranteed.
Student will be charged for meal plan during the review.
If any additional information or appointment is necessary, it needs to be completed within two weeks of the original request. Otherwise, the petition is voided.

Residential and Greek meal plans are a two-semester commitment. Plans cannot be canceled but can be changed up until the end of grace period (January 16 at 5pm).
Acceptable documentation (examples include):
Current financial statement or invoice from the organization
Invitation bid card. It needs to have student’s name and organization on it. Handwritten bid cards will not be accepted
Welcome letter/email. It needs to have student’s name and organization on it.
Acceptable documentation (examples include):
A letter from the international office regarding study abroad program/application
Letter from the school student will study at while abroad.
Please submit a letter from the company student will be working with during the Internship/Co-Op. Include dates of employment.

If reason is anything else than one of the options above, and student feels it is a valid reason for a cancelation exemption, please attach any supporting documents below and provide a brief explanation of the circumstance.
