Please bring the following items to your appointment:

• Completed Application, form DS-11, available at travel.state.gov
• Proof of U.S. citizenship – original or certified copy of your U.S. Birth Certificate, Certificate of Birth Abroad, or Certificate of Naturalization
• Photo Identification – Driver’s License, Government or State-issued ID card
• Passport Photo ($15.00 for 2 photos) – We offer passport photos onsite!
• Application Fee – Payable to U.S. Department of State by CHECK or MONEY ORDER only.
  • Amount vary depending on product & service requested. A fee schedule can be found here.
• Execution Fee ($35.00) – Payable to the University of Florida.
• Passports for Minors (Under 16 years of age) Both parents, along with the child, must be present with photo identification (Driver’s License, Government or State-Issued ID). See the parental consent section for additional information.
• Already Have A Passport? – If you already have a passport, you may be eligible to renew it by mail. Click here to see if you are eligible to renew by mail
